NOTICE OF PRIVACY PRACTICES

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

1. Understanding Your Health Record/Information

This notice describes the privacy practices of Flava Health Ventures LLC dba The Marque Longevity Lab and its staff (collectively, "Center"), and that of any provider with respect to your protected health information created while you are a patient at Center. This notice applies to Center, its providers, and any staff authorized to access your medical records. In addition, Center and its providers may share medical information with each other for treatment, payment, or health care operations described in this notice.

We maintain records of all care and services you receive at Center. Your medical information is personal, and we are committed to protecting it under the terms of this notice. This notice will tell you about the ways in which we may use and disclose your medical information. It also describes your rights and certain obligations we have regarding the use and disclosure of medical information.

2. Your Health Information Rights

Your health record is the property of the Center, but the information in it belongs to you. If someone has legal authority to make health care decisions for you, they may exercise these rights on your behalf. You may request restrictions on how your information is used or shared, though we are not required to agree unless you have paid in full out of pocket and ask us not to share information with your insurer. You may request a paper copy of this notice at any time. You may inspect and receive a copy of your health record, and we may charge a reasonable cost-based fee for copies. You may request an amendment to your record and if we deny the request, you may add a statement of disagreement. You may request an accounting of certain disclosures made in the past six years, and the first request in a twelve month period is free. You may also request that we communicate with you by a specific method or at a specific address or number and we will accommodate reasonable requests. 3. Our Responsibilities

We are required to maintain the privacy of your health information and to use or disclose only the minimum amount necessary to accomplish the intended purpose, except when it is needed for treatment. We will provide you with this notice of our legal duties and privacy practices, abide by the terms of this notice, and notify you if we are unable to agree to a requested restriction. We will also provide notice if there is a breach of your unsecured protected health information unless an exception applies. We may change our privacy practices at any time and the revised notice will apply to all information we maintain. A current version will always be available to you upon request.

4. Uses and Disclosures of Medical Information That Do Not Require Your Authorization

The following categories describe different ways that we may use and disclose medical information without your authorization. We will explain what we mean for each category, but not every use or disclosure will be listed. However, all the ways we are permitted to use and disclose information without your authorization fall within one of these categories.

5. Treatment

We will use your health information for treatment. This means we may share it with providers and staff involved in your care, or with other health care providers treating you, to coordinate and improve your treatment.

6. Payment

We will use your health information for payment. For example: A bill may be sent to you or a third-party payer. The information on or accompanying the bill may include information that identifies you, as well as your diagnosis, procedures and supplies used.

7. Health Care Operations

We will use your health information for regular health care operations. For example: We may use the information in your health record to assess the care and outcome in your case and others like it. This information will then be used in an effort to continually improve the quality and effectiveness of the health care and services we provide.

8. Other Uses and Disclosures Allowed by Law

We may use and disclose your health information as allowed by law. This includes sharing information with business associates who help us provide services, and we require them to protect your information. Unless you object, we may notify family members or others involved in your care about your condition and may share information relevant to their involvement. We may also disclose information for approved research, contact you with appointment reminders or treatment information, and communicate by phone, text, email, or patient portal unless you request another method. We may disclose information to public health authorities, report abuse or neglect as required, and share information for judicial, administrative, law enforcement, and health oversight purposes. We may disclose information if necessary to prevent a serious threat to health or safety, to comply with workers’ compensation laws, or to report issues related to medications or medical products to the Food and Drug Administration. We will also disclose information when required by federal, state, or local law.

9. When We Need Your Written Authorization

We will not use or disclose your health information without your written authorization, except as described in this notice. Additional circumstances that require your written authorization include uses and disclosures for marketing purposes and sale of your health information. We will always obtain your written authorization before using photographs of you for marketing purposes. See our Patient Consent for Photography and Image Use Policy. You may revoke any authorization in writing at any time, but this will not affect information we already shared with your permission.

If you believe your privacy rights have been violated, you can file a complaint with Center by contacting Flava Health Ventures LLC dba The Marque Longevity Lab at privacy@marquelongevitylab.com or with the U.S. Department of Health and Human Services Office for Civil Rights. There will be no retaliation for filing a complaint.

11. Effective Date and Changes to This Notice

This notice is effective as of December 1, 2025. We may change our policies and this notice at any time. If or when we change our notice, we will post the new notice at Center where it can be seen, and make copies available upon request.

12. Notice of Privacy Practices Acknowledgement

I, the undersigned, understand that under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), I have certain rights to privacy regarding my protected health information. I understand that this information can and will be used to: Conduct, plan and direct my treatment and follow-up among the multiple health care providers who may be involved in that treatment; Obtain payment from third-party payers; and Conduct normal health care operations, such as quality assessments and provider certifications.

I acknowledge that I have been provided the Notice of Privacy Practices containing a more complete description of the uses and disclosures of my health information. I understand that this organization has the right to change its Notice of Privacy Practices from time to time and that I may contact this organization at any time to obtain a current copy of the Notice of Privacy Practices.
I understand that I may request in writing that you restrict how my private information is used or disclosed to carry out treatment, payment or health care operations. I also understand you are not required to agree to my requested restrictions, but if you do agree, then you are bound to abide by such restrictions.

I understand that if I choose to share information about my care or results on social media or in public forums, Center cannot control how others use that information once I have made it public.
